liability waiver
& release

I, the undersigned, here by acknowledge that I have voluntarily applied to participate, or have elected to have my child, or a minor for whom I am responsible (hereinafter “my child”), to participate in services provided by MEMPHIS STEP AND SKATE LLC (dba "Step & Skate Movement"). By registering/booking myself or my child, I represent that I, or my child, is physically fit and able to participate in such activities. I am aware that participation in Step & Skate Movement services by my child or myself will involve my/my child’s participation in physical activities, and I hereby acknowledge that such activities can be dangerous. I hereby accept any and all risks of injury or death associated with such participation. 

 

In consideration of my participation in service provided by Step & Skate Movement, I hereby agree that I, on behalf of myself or my child, heirs, distributees, guardians, legal representatives and assigns will make no claim against, sue, attach the property of, or prosecute MEMPHIS STEP AND SKATE LLC or any of its officers, directors, agents, employees, contractor or affiliates of injury or damage resulting from negligence or any other acts, howsoever caused, by any employee, agent, or contractor of Step & Skate Movement, as a result of my participation, or the participation of my child, in any services or events facilitated by Step & Skate Movement. I have carefully read this agreement and fully understand and acknowledge its contents. I am aware that this is release of liability and a contract between myself and Step & Skate Movement and its affiliates.

Step & Skate Movement reserves the right to cancel or rearrange classes according to enrollment. Step & Skate Movement reserves the right to refuse business from a party or potential client for any reason. Parents/Guardians reserve the right to sit in on group or private skate lesson sessions as long as he/she/they are not causing a disruption or hindering other clients from learning. I understand Step & Skate Movement may take pictures/videos of the sessions and use them in advertising or on their website and social media.
